Check in — we'll be expecting you
Bring your photo ID, insurance card, and a list of your medications. If you filled out your forms at home, check-in takes about five minutes.
Meet your doctor and talk it through
Your doctor listens first — your symptoms, your goals, what you want to get back to. Then they explain your options in plain English.

Requesting medical records
Fill out the Records Release Form and send it to us by fax at (480) 371-1121 or drop it off at either office. You can also message us through the patient portal. Most requests are completed within a few business days.
