What to expect when you arrive

  1. Check in — we'll be expecting you

    Bring your photo ID, insurance card, and a list of your medications. If you filled out your forms at home, check-in takes about five minutes.

  2. Meet your doctor and talk it through

    Your doctor listens first — your symptoms, your goals, what you want to get back to. Then they explain your options in plain English.

  3. Get answers the same day

    Most testing, like ultrasound, happens right here in the office — often at the same visit. No separate trip to an imaging center.

  4. Leave with a plan

    You'll know what's going on, what we recommend, and what happens next — including what your insurance covers, verified before anything is scheduled.

Requesting medical records

Fill out the Records Release Form and send it to us by fax at (480) 371-1121 or drop it off at either office. You can also message us through the patient portal. Most requests are completed within a few business days.
